In order to begin a family law case or change an order in an existing family law case, you must prepare, file (and pay the filing fee), and serve upon the other party, certain documents. In addition to the above, Arizona has certain family court rules and counties have additional local rules that each party must follow. The list below is not all inclusive, but should be a great starting point.

Maricopa County Superior Court
Family Law Forms

The Maricopa County Superior Court provides several family law forms in addition to the form templates provided by the Arizona Supreme Court. The forms meet the Arizona requirements and may be used as templates for cases in other counties, provided that the appropriate county is stated and local county rules are followed.
